What are Pre-Enrollment Supports?

Pre-enrollment supports are the people who help you enroll in the SDP program. They are paid for by the regional center. There are two types of supports you can get: Person-Centered-Planning and Self-Directed Supports. The person who provides these services is often called an Independent Facilitator (IF).

You get to select who you want to support you.

Person-Centered Planner

A Person-Centered Planner helps you create a plan for your life based on what matters most to you—your goals, values, and dreams. In the Self-Determination Program, they guide you through the Person-Centered Planning (PCP) process, which is the first step before creating your budget and choosing services.

What They Do:

Listen to you and help you explore your strengths, interests, and needs
Facilitate meetings with your support team (family, friends, providers)
Help write your Person-Centered Plan, which becomes the foundation for your services
May work with your Independent Facilitator or be the same person, if qualified
A Person-Centered Planner doesn’t make decisions for you—they help you make your own choices.

You are not required to have a person-centered plan to enroll in SDP but it is highly recommended. Your entire program will be built on the foundation of this plan. It will help you determine what services you need to build your budget. They will guide you in how to determine what should go in your spending plan. A Person-Centered Planner can earn up to $1000 per plan. There is no set hourly rate. They must show they have training.

Self-Directed Support

After your person-centered plan is done, a Self-Directed Support provider will help you finish the transition.

They can offer these services:

Support you during budget meetings
Make sure your goals are in your IPP
Help choose your Financial Management Service (FMS)
Create your spending plan
Connect you to local services and providers
Help set up new services or employees
Make sure everything is ready for your transition day

Once you choose your support provider, tell your service coordinator. The service will be added to your IPP. You can get up to 40 hours of help, and the rate is around $50/hour, depending on your regional center.
