Person-Centered Plan

A person-centered plan (PCP) helps you create a life that fits you. It focuses on what matters to you—your hopes, dreams, and goals. The plan shows the steps to reach them and becomes the foundation for building your self-determined life.
Orientation

The first step is to attend an orientation—either through your regional center or online with the State Council on Developmental Disabilities (SCDD).

Pre-Enrollment Support

The regional center will pay for someone to help you get started. You get to choose who helps you. This person is usually called an Independent Facilitator.
Budget

The Self-Determination Program gives you a budget to use for your services and supports. The amount is based on what the regional center paid for in the last 12 months, any new needs you have, or changes in your situation.
Spending Plan

Spending Plan: You create a spending plan to show how you will use your money in the Self-Determination Program. It’s your action plan for reaching your goals. This is where you take charge and make your plan real!
Financial Management Service

In the Self-Determination Program, the Financial Management Service (FMS) handles the money and pays for your services. They are an important partner, so it’s key to pick one that meets your needs. This is the only service in SDP that is required and must be a regional center vendor.
